Emailmeform.com is one of the providers we have used before. They offer genuinely free forms with spam protection, and allow each free user account to have five forms. The free forms is hosted in their server but you have total control over the email address and addresses that the forms are sent to. Once a user completes a form the form information is sent via email to the email addresses specified.

The only drawback is that we did experience a server failure for 24 hours, during which time we did not receive any forms.
